This is an image of the monument from near the Warsaw old town. ;The main dedication says

Tym kanalem po bohaterskiej obronie Starego Miasta - Starowki prezeslo do Sródmiescia i na Zoliborz 5300 powstańców Grupy Polnoc 1944 . 2 września . 1974(Poland)
Using this sewer, after their heroic defense of the Old Town, 5300 fighters from the northern group escaped to the city centre and Zoliborz 1944 . 2nd September 1974 (my translation)

The monument dates from 1974 at which point, due to polices of the Soviet controlled communist government it had become possible to have monuments to those who fought in the Warsaw Uprising and to mention that they were insurgents, but it was still not possible to say that most of those insurgents were members of the Home Army (Armia Krajowa).
